Instructions for ordering a Death Certificate by mail.
 
Step1     Complete the PDF application here -          Death Application
    The PDF application can be filled out online and printed, or you can print
    the form first and fill it out by hand. 
 
Step 2 Very Important
 
Take the application to a Notary and have the application form Notarized.
 
Step 3    
 Mail in the Notarized application with a check or money order to:
