Instructions for Ordering a Death Certificate on-line

 
STEP 1
Download the application here. Death Certificate Application
 
 
The PDF application can be filled out on-line and printed, or you can print the form first and fill it out by hand. 
 
 
STEP 2 Very Important
Take the document to a Notary and have the form Notarized.
 
 
STEP 3
Go to our on-line order form page at the link below, to order enter the document and billing information.

FINAL STEP
Fax the notarized copy of the Death Certificate Application to either of the following numbers within 10 calendar days:

Please do not fax any form of Identification.
Please Note: All Internet Orders will be held for 10 Calendar days. Any orders that have not been completed in the 10 day period will be considered cancelled, and you must resubmit your Internet Order form and re-fax your notarized copy.
 
To apply for birth records online, you must complete the Application for Certified Copy and the form must be notarized. If you mail your application and it is not notarized, your application will be rejected as incomplete and it will be returned to you without being processed.
